About this product

A health questionnaire, a signed waiver and an emergency contact are the basics your insurer will expect you to have for every client.

One row per client. Anything missing is flagged NEEDS ACTION. Health questionnaires are renewed every 12 months by default - your own policy, or your insurer's - and the client is asked to update theirs 14 days before it runs out, with a link to your form if you have one.
